Do you except Medicare assignment?

Yes, this means you are only responsible for your deductible and your coinsurance. The deductible is once a year it is $100.00. Coinsurance is 20% of the allowed amount after assignment.

What plans do you participate with?

We participate with numerous plans it is always best to contact your carrier and give them our information. Just call the number on your member ID card or refer to your provider directory. Many of the plans now have this information on the web. (SEE LIST)

How much do I have to pay at the time of my visit?

This depends on your plan. Usually it is your deductible (if you have one) and/or your co-pay or coinsurance. To get this information before your visit we try and verify your coverage this gives us an estimate prior to you seeing the doctor. However this doesn’t guarantee payment by your carrier.

What do I need to bring to my visit?

Your insurance card or cards, picture ID and a list of your medications and any recent office notes or test that may be helpful too your treatment.

What information do I need to make an appointment?

We will need the general information name, phone number, reason for your visit etc. We also need the information off of your insurance card in order to verify your coverage. The call goes quicker if you have your ID card in front of you at the time of your call.

Yes

How long before I can get an appointment to see Dr. Alexander?

Allow 24 to 48 hours for an appointment but exceptions are made for emergencies.
